The Federated States of Micronesia present capacity gaps in cancer care stemming from four widely separated statesYap, Chuuk, Pohnpei, and Kosraeeach operating independent health departments with no shared electronic health record. The national hospital in Pohnpei holds the only CT scanner; patients from Chuuk or Yap must travel by field-trip ship for up to 10 days. Micronesian Cancer Care funding therefore prioritizes infrastructure that can function without reliable broadband or consistent electricity.
Workforce constraints are severe: the four states together employ fewer than 10 physicians with formal oncology training. Chuuk State, the most populous, has recorded the highest cervical cancer mortality in the U.S.-affiliated Pacific, yet possesses no colposcopy equipment. Funded projects must therefore budget for solar-powered diagnostic kits and for training Chuukese nurses who can perform visual inspection with acetic acid.
Transportation infrastructure relies on weekly flights among state capitals and monthly supply ships to outer islands. Any equipment order must fit within the cargo limits of a Twin Otter aircraft or be shipped via Honolulu, adding 60–90 days. Proposals that assume real-time telemedicine links are rejected; reviewers require contingency plans using Iridium phones and paper-based tracking forms.
Demographic data reveal that 60 percent of the 105,000 residents live on outer islands with no road access. Capacity-building plans must therefore include state-specific memoranda of understanding that allocate a share of training slots to each state rather than concentrating resources in Pohnpei. Unlike Palau applications, FSM proposals must address four distinct language groups and four separate legislative appropriations processes.
Successful Micronesian Cancer Care awards document prior delivery of cold-chain pharmaceuticals to at least two outer islands in each state and include audited fuel logs for the preceding 24 months.
