The Federated States of Micronesia operates four state hospitals and 110 community health centers spread across 607 islands, with 65 percent of the 115,000 residents living more than 50 kilometers from the nearest physician. The country’s 2,700-kilometer east-west span creates persistent workforce and supply-chain gaps that neighboring island nations address through different geographic arrangements.
Chuuk and Yap states report vacancy rates above 40 percent for nurses and community health workers, while Pohnpei’s central hospital must serve as the referral hub for patients flown from outer islands under a single weekly air service. Broadband penetration below 30 percent in remote atolls prevents routine use of telehealth platforms.
The national health workforce development plan sets a target of training 200 additional community health workers by 2028, yet current training capacity is limited to one cohort of 15 students per year at the College of Micronesia. Infrastructure projects must account for the absence of deep-water ports on most outer islands.
This funding supports proposals that expand training pipelines or construct modular clinic facilities designed for transport by the country’s inter-island cargo vessels. Applicants must provide letters from the relevant state health department confirming alignment with the current workforce shortage list.
Strong applications include contingency plans for equipment maintenance that rely on spare parts stocked in Pohnpei rather than frequent international shipments. Unlike applications focused on the Marshall Islands, reviewers expect explicit accounting for the four-state governance structure and separate state health budgets.
